If you are interested in having POP visit your company, please review the following information:
- Cost per patient is $48.00 (cash, check, or credit card accepted) unless covered by insurance co-payments, company wellness or flex account.
- Your company is required to pay a $100.00 site-fee for POP for travel to any location within Maricopa County. All other locations outside Maricopa County require a negotiated site fee. The fee will be determined by the distance and travel time required.
- Minimum requirement per session is forty (40) screenings.
- Is there a 50 AMP electrical outlet available at the trailer-parking site?
- If it is hot, will the POP trailer be close to an indoor entrance for registration?
- Can the trailer be delivered the evening before the screening? Will there be cones to alert the driver?
- Please send a postcard/email to all employees to place the prostate exam date on their calendars.
- Provide your employees advertisements of the screening dates via e-mail, payroll stuffers, and the ”POP’s” continuous DVD prostate health media loop.
- Posters and reminders should be displayed in all men’s restrooms.
